  • Go to official website of Chief Electoral Officer, UP at
  • Download the Form 6 for New Voter Registration
  • You can also download Form 6 from this link directly.
  • If you want to enroll your name in voter list by overseas elector, you can download Form 6A.
  • Attach any of the identity proof
  • You can attach ration card copy/driving license copy/passport or any other government approved document. 
  • For address proof, do attach telephone bill or electricity bill or gas connection bill
    Duly filled Form 6 can be submitted in nearest polling station or nearest government office for electoral process.
